It's quite a bit different from Zelda. All that's the same is the map and item displays. And bombs.
Besides, Zelda focuses on exploring multiple dungeons across the overworld, getting an item in each one.
Isaac has only 1 dungeon, with an item each floor. The charm of the game is that the layout is randomly generated, and every playthrough is unique. You could have an amazing playthrough with 3 overpowered powerups on the first 3 floors, or you could have a really difficult playthrough with no useful items until the last floor.
Oh, and there's the whole fact that Binding if Isaac is a little more...gory
